How a disease will affect the following levels of the organization, namely the organism, their organ systems, organs, tissue, and the cell will depend on what type of disease and how damaging it is.  

When an organism becomes infected with something, it always starts at a cellular level. If that happens to be anywhere near a vital organ, then we can expect a slow and unnoticeable destruction to take place. What started from a few cells would eventually damage tissues. And then the organ itself. Usually, when one organ is damaged, another one will follow since these are all connected through a system that needs to work together, one organ takes oxygen, the other one pumps blood throughout the body to deliver nutrients and oxygen, and we all know that the kidneys keep our blood clean as it circles around inside us. So you can see how it all works like a clock and maintaining it is important for survival.  

If one organ completely fails, it is usually just a matter of time before the unlucky organism succumbs to a slow decline and death. There are ways to live through a completely ruined organ, but those are not options for the less wealthy among us, and much less for other organisms.

To explain how disease affects each of the following organization let us explain first understand the biological level of organization.

From the bottom to the top we have (1) cell, the basic unit of life, (2) tissue, that were composed of cells that have same functions and appearance, (3) organ, that is composed of different tissues that have the same functions and lastly the organ system that is composed of different organs that functions together.  

If the cell is damaged, the tissues, organs and organ system will fail to perform its functions. Since cell is the foundation of the organ system.

To better illustrate it, let us have an example, the cells of the stomach is responsible for the breaking down protein into amino acid. One of its cell is responsible for the production of pepsin and pepsin together with the HCl is responsible for the breakdown of protein into amino acid. If this cell is damaged, protein will not be broken down into amino acid, as well as the acidity of HCl will not be controlled leading to the damaged in the lining of the stomach. Thus, giving further damaged to the tissues lining the inner of the stomach. With this scenario, the stomach could be having more damaged and possibly will not perform its functions. Since stomach is one of the organ of digestive system, this will be a failure in one of the function of digestive system that will lead to a lot of complication.
